LOSING A WALLET
DO

Place
the contents of your wallet on a photocopy machine;
photocopy both sides of each license, credit card, etc. In
the event your wallet is lost or stolen, you will know
what you had in your wallet and all of the account numbers
and phone numbers to call and cancel.
Keep the photocopy in a safe place.
Here's some critical
information to limit the damage in case you lose your
wallet:
1. Cancel your credit cards immediately -
the key is having the toll free numbers and your card
numbers handy so you know whom to call.
(a) Contact your bank(s) or financial institution(s) and report your
missing bank and /or credit cards
(b) Notify the Customer Service or Credit Department
if you have lost a retail credit card or any other
credit card.

File a police report immediately in the
jurisdiction where it was stolen or lost, this proves to
credit providers you were diligent or in case it is turned
in. Do NOT dial 911. Refer to you phone book for the local
telephone number or dial 1-8888-310-1122. This is also an
important step with regard to your Driver’s Licence.
File a
report with O.P.P. Anti-Rackets PhoneBusters:
1-888-495-8501.
3. Call the three national credit reporting
organizations immediately to place a fraud alert on
your name and SIN#. The alert means any company that
checks your credit knows your information was stolen and
they have to contact you by phone to authorize new credit. The
numbers are:

Contact government offices to
report your lost government identification and cards:
Canadian
Citizenship Certificate: 2
pieces of ID and
2 Citizenship sized photograghs are required when applying
for a citizenship certificate. Complete
‘Application for a Citizenship Certificate from Inside
Canada’ form. If a name has changed, proof of the change
must also be provided. If possible, provide a Citizenship
certificate. Replacement time is 3-4 months, but faster if
applying in person, but first call for an appointment.:

Canadian
Passport: You
will need to fill a ‘Statutory Declaration’ form -
Form PPTC 203 – which needs to be
notarized. Report your passport missing by contacting police
and by calling: 1 800 567-6868. To replace
your lost or stolen passport, you must reapply as if
applying for a passport for the first time. You will need to submit the passport application form
with 2 identical photos and an original document proving
Canadian citizenship. If you find your passport that was
reported lost or stolen, immediately advise the Canadian
Passport Office.

(Senior’s card): Call 1-800-277-9914 – You will need your full
name, SIN#, mailing address.
Ontario
Birth Certificate:
This is the most important piece of
identification to replace first since it will be needed to
obtain your other lost or stolen documents. (Application
found in this folder). Call 1-800-461-2156. ID is required
– example: Driver’s Licence. Apply on-line & you
are guaranteed to receive your certificate in 15 business
days of
application through Service Ontario through the following
link: https://www.orgforms.gov.on.ca/eForms/start.do
or www.cbs.gov.on.ca.
Or you can mail the form ‘Request for Birth
Certificates’ to: Office of the
Registrar General in Thunder Bay for processing,
which takes 6-8 weeks to process. Or your application can
be faxed to 807-343-7459 with a credit card number.
Completed applications for expedited service may be faxed
to : (807) 343-7411. Applications must include proof of
urgency and provide payment by credit card: Visa,
MasterCard or American Express. (In order to qualify for
expedited or emergency service, you must provide proof of
urgency. This proof may include: A medical emergency; a
letter confirming new employment; a letter from a
consulate or embassy confirming an appointment; airline
tickets; a letter confirming an appointment for an
immigration hearing; travel reservations; a wedding
invitation, where the applicant is the bride or groom; a
letter from an RESP dealer. Other situations are subject
to review. Please be aware that a guarantor is no longer
required to sign the birth certificate applications. To check on the status of your application go to: http://www.orgforms.gov.on.ca/status/start.do.
Report a lost or stolen Birth Certificate to the Office of
the Registrar General
(ORG) immediately, by calling 1-800-461-2156. Your
lost birth certificate will be cancelled so no one else
can use it. If you find a birth certificate, notify the
ORG or deliver it to the police at once.
Ontario
Driver’s Licence: Please see a staff member at any
local Vehicle Licensing Issuing Office in your area. You
will require 2 pieces of identification. Example: birth certificate, ID proof showing your
legal name & signature. A temporary license is issued
immediately and a permanent one will be sent by mail
within 4-6 weeks. For more information contact the
Ministry of Transportation at 1-800-387-3445 or the
Ontario Government Information Centre: 519-873-4000.
Ontario
Health Card (O.H.I.P.): You
can make your request by telephone as long as you have not
changed any information as shown on the card or the card
has not expired. For more information call: 1-800-664-8988
or 1-800-268-1154. When applying in person, three pieces
of ID required to prove citizenship/immigration status,
residency in Ontario and identity: Example: Driver’s
Licence and Birth Certificate, Employer record, monthly
mailed bank account statements; mortgage, rental or lease
agreement or utility bill; Ontario Motor Vehicle Permit or
credit card are usually accepted (refer to form 9998E-82).
You may be asked for
additional documents. For example, if your name has
changed you may need to provide a marriage certificate or
a change of name certificate. You will receive a temporary
health card immediately and a new one will be sent by
mail.
Ontario
Vehicle Registration: See a staff member at any local
Vehicle Licensing Issuing Office in your area. The owner
of the vehicle is the only one that may apply for the
replacement. The application form requires the plate
number of the owner’s vehicle or the VIN (Vehicle
Identification Number) of the vehicle. A permanent card is
issued immediately.

Card: For

Social
Insurance Number: Contact police and ask for a copy of
the police report and case number. Contact SIN Call Centre
at 1-800-206-7218 and select option ‘3’. Complete
application form on-line or for faster service apply in
person (at Human Resources Skills Development Canada, 120
Queens Avenue, London) as it does not require you to part
with your valuable identity documents. You will require
original documents that proves your identity and status in
Canada. Example: birth certificate, passport or baptismal
certificate. Married women who use their husband’s name
need a copy of their marriage certificate.
